Let's ignore the fact that Ant-Man would have to wait around for ages until Thanos goes to the bathroom because I swear the dude never takes a bathroom break in any of the movies or comics. If we consider the physical attributes of the characters, we can see that Thanos is significantly taller than Iron-Man.

Furthermore, Scott Lang, who is known as Ant-Man, is a little bit shorter than Tony Stark at his normal size. This means that if Ant-Man were to venture inside of Thanos's butthole, he wouldn't do much damage if he was standing upright at the same angle as the Titan. The body would severely hurt and would definitely result in the breaking of the bowels and misplacement of several different organs. However, given Thanos's immense power, he could likely overcome such injuries.

If Ant-Man were to regrow to his normal size, positioning himself sideways to target the lower regions and stomach of Thanos, the impact would be more severe. Coming out at an angle would definitely rip up more skin, but it's highly unlikely this would kill Thanos if he has the Infinity Gauntlet and can regenerate his body after seeing the damage.

No matter the angle or method, if Ant-Man regrew to this size, the end result would be a serious injury, but likely not fatal. The phrase 'No more Snappy Boi' encapsulates the irreparability of such actions.

Conclusion: While the idea is entertaining, it doesn't hold up to scientific scrutiny and relies on the comic book storytelling tradition of pushing the boundaries of reality for the sake of an exciting storyline.
