In a real-life scenario, the Joker wouldn't have the advantage of plot armor or heroic assets to rely on. Furthermore, he wouldn't be held back by Batman's fictional prowess. Factors like public outcry, law enforcement actions, and the judicature would ensure that the Joker would be brought to justice. These institutions function differently in the real world and would pose a significant threat to the Joker's reign.
